Protocol Deviation

Any departure from the IRB-approved protocol, whether intentional or unintentional.

Protocol deviations range from minor administrative slips to major deviations that affect subject safety or data integrity. Major deviations and those that increase risk or affect subject rights typically must be reported promptly to the IRB and sponsor.

A pattern of repeated minor deviations can itself constitute a reportable issue.
